Terms and conditions for Silver Membership

You will be billed on the 1st of every month

Hold Policy

*Effective May 1st, 2018 holds may be placed for Injury, extended illness, or pregnancy/maternity leave ONLY. We will NOT place a hold for travel or holidays.
*To place your membership on hold, you will be charged a one time $20 fee that will be credited towards your account when you return. Please contact us directly at jaclynrobertson@fit4mom.com if you are holding your membership while on maternity leave or for an injury.
*Form must be completed 15 days before your autobill date (a minimum of 15 days notice is required)
*Your membership can be on hold for up to 4 months per year.
*All hold periods will be set up for 4 months unless otherwise requested.
*Dues payments automatically resume at the end of the hold period. We do not send notification prior to the payments resuming. It is imperative that you stay on top of your membership status with jaclynrobertson@fit4mom.com if you have any questions about the duration of your hold.
*To return prior to your hold period ending, email jaclynrobertson@fit4mom.com with your exact date of return to class. This will be the first day that your dues payments resume. It can be on any day of the month and the dues will be pro-rated.
*To extend your hold period, you can email jaclynrobertson@fit4mom.com by the 15th day of the month (or 2 weeks notice) prior to your dues payments being scheduled to resume so that we can adjust your hold period before your dues payment happens.
*To attend classes while your membership is on hold, you can pay the rate of $25/class. Just let your instructor know at sign in.

Cancellation Policy

*Your request must be filled out 30 days before your autobill date (your autobill date is the first)
*You will receive notification of your cancel request before the end of your last month of classes.

*Our memberships have a three month commitment. If you cancel before your commitment period is over, you must pay the remaining balance.

*You can downgrade or upgrade your membership with no notice OR convert to a pass with no notice.

*We will not go back and credit dues once they have been paid. All requests are for future months only.
